It is basically just like garden chemical sprayer with a gauge and a cap for the master cylinder, instead of a spray nozzle. You pour in a couple of quarts of brake fluid, replace the master cylinder cap with the one connected to the hose and pump it up to around 15 lbs. When you open a bleed valve, out comes air and dirty brake fluid until it runs clean, through an attached hose and into a catch bottle. Since there is constant pressure, it doesn't back up into the brakes.
