If your income is lower than 400% of poverty line, you can get a subsidy. And the subsidy is quite big if you have zero income. But it's still different than MediCare. MediCare is totally free, but for the insurance from exchange, even though the subsidy could cover most of the premium, you still need to pay some co-pay, deductable etc when you go to see a doctor.

For the cost of premium, it depends where do you live. Some states have their own exchanges, some refuses to set up their own so people in those states have to rely on the exchange market set up by federal government. From the released premium cost in California, the premium in average is actually lower than expected. If you are young male without pre-existing condition, you will probably pay more than right now; but for people in 40s or 50s, the premium will be lower than it is, assuming we are talking policies with similar coverage.
