车祸后求帮助

前一阵发生车祸, 是对方全责任, 当时叫了救护车. 现在对方保险公司发信说提供一次性$1000, 需要签一份文件, 声明以后不可以对保险公司,对方及其对方后人, 保险人所在公司提出任何诉讼,即使以后发现身体上有任何和车祸有关的病情.请问我应该签吗? 我有什么权利吗? 如何保护自己呢? 我现在没有感觉什么,除了当时头晕,心跳过速等外. 谢谢任何善意的回答和帮助!
