qestions about my mom's 485 form filing

in part 3 B,about her relatives,

1. I am the petitioner, should I be listed in?

2. My dad has passed away, she he be listed as current hu*****and? (same question for G-325)

3. for different translation of her first name, such as xxx'y , xxx y or xxxy on different supporting documents, do I need to get a noterization affidavit?
