Question 4: Consulate where immigration visa or USCIS offices where adjustment of status were granted?

-leave it blank or put "N/A" unless you got the green card by getting immigration visa in China.

- You missed the "or" in above question  (I-90)


-Thanks, I did miss that. Put the USCIS Service Center there such as TSC or NSC which approved your green card.
