AP question

I will back to China at May, but have some question for using ap.

1: I have two I94. One is white card I got it when I enter US last time as a F1 student. Another is attatched with my H1-B approval notice.

So Which one should I sumbit?

2: Acoording to PJiang's blog, AP issued by NSC have 3 pages. But mine form NSC only has two pages. Will this casue any problem?
