From the point of view of the state

it is a necessity.

In terms of health insurance, people without insurance (usually poor people) will use medical service (e.g.emergency room) and accrue liability--it is also a liability.  The real question is which way is more costly (to the nation as a whole)? --not a subject that can be concluded in a post.
