between the insurance company and doc's group.  If the doc's group gives you a discount for no reason, the insurance company could see this as a breach of the contract.  It is complicated.  If you have unexpected financial difficulties, you can beg them to write the whole thing off, but otherwise you need to pay. BTW, people with high deductable insurances usually also have HSA to pay for the deductables.  At least you get a tax break.
