Bilateral frontal lobe bleeding. A lot of edematous change in the entire brain. Enlarged ventricles.
I have to say this is one of the worst intracranial bleeding I have ever seen by myself (excluding textbook).
Prognosis is bad. Patient is breathing on ventilator that gives enough oxygen for heart. Life quality will be very poor even if he can survive this one, which I even doubt.
I am sorry about that. If this were my own patient, simply based on this scan and the information you have given early, I would call family to discuss code status. Management is reasonable now: fluid, nutrition, ventilation, antibiotics if for infection. I do not think neurosurgeon is able to do much, although I am not a neurosurgeon.
