The documents for 485 application is different from 140.

As to 出生证明,

If you have birth certifate sheet from the hospital where you were born, it will be great.

If not (for most from mainland), you need to get 出生证明 from your employer in China or local government, then go to Notary Office to get it notarized. Then the notarized copies with your 485 application. That is what most people did. I have not heard anybody got problem with that.
