In general, the applicant should get at least one with 3 IIs if the applicant could convince the interviewers his/her suitable for a physician (MD only and MD/PHD) and a scientist (MD/PHD).  As an MD/PHD applicant, the challenge is that the applicant has to satisfy both physician and (physician-)scientist interviewers. A genuine passion for seeing patients and the deeply understanding of the applicant's research are equally important.
