国内的重大疾病保险与美国的人寿保险有什么不同？

最近看了几个什么中年的流感，里面提到国内的医保不报一些重大疾病，这个时候就要用另外购买的重大疾病保险填上那动辄上几十万的费用，特别是ICU的费用，请问美国的人寿保险包括的重大疾病可以提前支付，这两种保险有啥不同，美国的便宜多了。
