For hotel reservation, you must get a confirmation directly from italy to you.
The Italy Consulate doesn't take confirmation from a third party website. I booked my lodging in one hotel for the whole duration in Italy.Then I called the hotel and asked for an email confirmation.
I didn't book lodging according to my itinerary, because otherwise I had to show 9 email confirmations. I didn't have time and efforts.
After I got the visa, I cancelled the unnecessary staying. I was charged 10% deposit and lost that amount of money, though.
Insurance is important too. I bought insurance online. It is not expensive at all, only $30.
