请教8老师申请美国护照

申请孩子新护照，被要求追加材料。以前的旧护照（11岁时的）已经过期。这次是17岁，所以在邮局交了旧护照和孩子的驾照。今天收到信： 

Please submit a notarized written statement of consent to the issuance of your passport from either of your parent (s) / guardian (s). Also include a clear photocopy (front and back)of your parent's /guardian 's identification, such as: driver license; passport book or card....

请问这个notarized written statement 是自己写几句话就可以吗？

当时我们两个和孩子都到了邮局，好像当时没问我们什么，就把孩子的材料收进去了。现在是圣诞节的飞机票已经买了。真是着急。
