I ever heard that the relatives might testify the birth date and birth place in lieu of a formal certificate in the case that there is no such stuff available at all. I am not sure of it, however. The best way is to consult an attourney or dedicate to looking into USCIS's website.

By the way, how does people living in countryside get their passport in China? Thanks.
