8老师好，我为父亲同时递交I130,I485,所以麻烦再问几个问题：
1）. I-130，Part5，我两年前已为母亲申请了130，所以question1是 Yes.
Provide name,place,date of filling: 这里是不是填母亲的姓名，我的Physical Address ？date filled 是不是母亲I130的 received date (或 proirity date) ？
2）. I485's Evidence of continuously maintaining Lawful status since arrival in US:
Instructions says: Include evidence for every time you entered the United States and for the time periods spent in the United States.
我爸从2011至今入境了5次，这样我需要把过去每一次的记录都要附上？ 还是只用most recent I-94就可以？
