My case is：I am green card holder and I am filing 485/131/765 for my 4-year son (I filed 130 for my son and the 130 was approved and the PD is current now). I completed all forms for him.

Here are my questions:

1. 485 form,

    Part 10: Applicant’s statement, contact,… and Signature”
        1a, “I can read and understand …”    DO I NEED TO CHECK THIS?
-No, you need to select "2. At my request...."

          2 “at my request, the preparer name in part 12 ___prepared this application for me ..”  DO I NEED TO PUT MY NAME HERE？-Yes
    Part 12: Contact Information, …., of Person Preparing this Application, if Other than the applicant”  DO I NEED TO COMPLETE THIS?-Yes
Because your son is under 14 years old,, you also need to sign the form for him in Part 10, Item 6.a. and 6.b. Applicant's Signature and Date of Signature" part using this format "your son's name/parent: your name; e.g. XXX/Parent: YYY".

2. I-864 form:
For “IRS tax return transcript”, DOES IT MEAN THE “transcript we print from IRS website” ?  I just login the IRS website and  print the copy of the transcript from IRS website, IS THIS ENOUGH?
-Yes.

DO I NEED THE COPY OF 1040, W2 WE FILED TO IRS?-Not need if you provide the transcript.
