10.  Form I-693  Report of Medical Examination and Vaccination Record (sealed in the original envelop by the civil surgeon)

11.  Form I-864 (Affidavit of Support)

 

    a. Tax returns for 3 years

    b. W-2's for 3 years

    c. Employment letter ( from petitioner spouse's company)

    d. Pay stubs for petitioner spouse

12. Form I -864 A

-Provide the copy of you and your spouse's marriage certificate
