1. G-1145 form -Not need it because you file it with I-130 (list both I-130 and I-485 in G-1145).

5. 母亲的出生公证书复印件

6. 母亲中国护照复印件-Including the visa stamp page

7. 母亲的 I-94

10. Employee status and income verification letter from my employer

11. 2014 全家 Tax return form 1040 复印件 -You had better rpovide the copy fo the tax return of 2015 with W-2. If not now, USCIS may send the RFE to you for it later, then you can provide it at that time.

13. last three month pay stub

问题：

1. 照片是我母亲的照片，不是我的，对吧？（感觉问的很愚蠢。）-Yes.

2. Tax return 和 W2 要最近一年的copy

-This is required.

还是最近三年的？

-This is optional.
