请问Eb1a补材料，还需要填写什么表格之类的吗？

我收到通知单子上这么写：

但是后面又写了这样一段，不明白这个G-28和我有关系吗？
Please check the appropriate box regarding if there is a new Form G-28. Notice of Entry of Appearance as attorney or 

accredited Representative. Please place the new Form G-28 directly under this sheet.
