我们的排期终于到了，现在要去体检，请问8老师：

1。我们需要带什么表格去体检？
-I-693.
2。医生会给我们一个体检报告么？
-Yes, the Dr. will complete the I-693 with the physical exam results.

主，副申请人的体检报告是分开的么？
-Yes. Each person has own personal report.

这个体检报告是要和485表格一起寄走么？
-Yes.

3。我曾经在国内打过疫苗，但是那个疫苗记录本找不到了，需要重新打疫苗么？
-Probably need to do it again because you don't have the record.
