请教父母移民130 RFE中，Booklet issued by the Province's Health Departme

父母移民，收到要求提供几个额外证件：
1.Birth Certificate
2.Hospital Certification
3. Family House hold registry
4. Family House hold register form
5. Booklet issued by  the Province's Health Department regarding the birth.

1和2肯定没有，因为我没在医院出生。户口本的话，我的和父母不在一个户头上了。关于4，公安局那边说原始的户口登记表都查不到了，因为时隔太久了。那个5是什么意思？国内卫生部有每个人的出生记录吗？
