Help!!! I am a F-1 in U.S.

and I am applying for Canada permanent residence. Need you help on following questions:

1. In form IMM 0008 Generic, what should I answer to current contry of residence and other countries with resident status? Should be U.S., China, or China, China?

2. How can I get the marriage certificate notary?

3. The company I worked for already went to bankruptcy, and that was the only one company I worked for. What can I do?
