I am afraid that I am not professional for this question. But I do heard that you need to bring the following documents with you:

485 receipt, medical proof signed by the doctor, personal documents such as passport,driver's liscence and etc. Before you go, make sure you double check with the local office through their website, or by calling them; or you can ask somebody who has applied such an AP previously.
