I don't think you need either of them. My parents got thier visa lastJuly and I didn't prepare these forms. I prepared the following:

1. A letter to the Consulate stating that I am inviting my parents to visit me and will be responsible financially for them white they are here.

2. A pay stub indicating financial capability to privide these support.

3. A letter to the Consulate in the name of my parents stating the reason why this visit is temperary.
