I think you mean your H1B status got approved. Now, you want to apply for H1B visa. There is a big difference between visa and status.
To apply for H1B visa, you don't need DS158. It's for F, J, and M visa applicatns. If you are a male between 16 and 45, however, you do need to fill out DS157.
On items 32 and 33 of DS156, since youa are on H1B not F1 you just reply yes(for #32) and no(for #33).
