Help needed

I recieved a letter from Buffalo after I have finish my ME stating: "You have given me insufficient information in your application t allow me to complete mandatory background cheks. Therefore, I have stopped all processing of your application. Please forward the following informaiton ASAP, but no later than 60 days from the date of this letter" The period required is from May 2002 to present.
1. List details of your personal history showing employment, study and any other use of time. Include dates, employer, occupation and location for the priod.
2. List details of your residence history
3. List all groups?organizaions of which you have been a member including military service/rank.
Anyone has recieved the same letter before?
