FINDINGS: A cystic mass in the head and uncinate process of the pancreas measures 4.2 x 2.9 cm in the axial plane versus 6.9 x 6.4 cm in 2015. On the prior examination the lesion was unilocular and did not appear to communicate with the main pancreatic duct.

 

IMPRESSION: Since 2015 the cystic pancreatic head mass has changed from a large unilocular lesion to a smaller multiloculated lesion causing biliary and pancreatic ductal dilatation and mild atrophy of the distal pancreas. The most likely diagnosis is intraductal papillary mucinous neoplasm. The differential diagnosis is serous cystadenoma. Surgical consultation is recommended.
