PERM用的是公司一个地址（现已搬家，失效），最近的H1B用的是另外一个（和旧地址在一个州但是不在一个城市所以prevailing wage还是有差别的，另外PERM和H1B的 wage level也不同），求问G325a的表或者485其他要填地址的地方应该和哪个一致。
-Put the actual addresses of the last five years your worked there even if it is the same company, but different locations you worked.
