请教8老师： H4 EAD 申请材料

8 老师您好：

我在USCIS网站上看到以下证明身份的材料清单（https://www.uscis.gov/working-united-states/temporary-workers/employment-authorization-certain-h-4-dependent-spouses#How%20to%20Apply）：

我不确定要求中“OR" 的意思：

是需要提供1&2&3 项加上 4或5 中的任意一项？
还是只提供5项中任意一项即可？

另外请问第4项：美国使馆发的H-4 签证可以吗？

还有请问第5项：是指中国身份证吗？

Evidence of : A government-issued identification document with photo	A copy of your last EAD (if any); A copy of the biometric page of your passport; A birth certificate with photo ID; A visa issued by a foreign consulate; or A national identity document with photo.
