My question is: If I have applied OPT will I say 'YES'? and I will attach OPT copy and approval letter?
Same situation with me, I selected YES and attached the copy of card (front and back) and approval notice.

List including: Form 765, Copy of 485 receipt, copy of i-94 (current). OPT copy, passport copy,2 photoes, check. Anything else?
My list is pretty simple:
1.Check of $340
2.Form I-765
3.Copy of I-485 receipt notice
4.Copy of front/back of previous EAD (OPT)
5.Copy of Approval Notice of previous EAD
6.Two photos
But it won't hurt if you attach copy of I-94 and passport.

If you are filling form I-765 alone, the address is correct.
