Hi, I have some questions on I-765.

My question is: Will be same A number in 485?

line 11. Have you ever applied Employment Authorization From USCIS?
My question is: If I have applied OPT will I say 'YES'? and I will attach OPT copy and approval letter?

List including: Form 765, Copy of 485 receipt, copy of i-94 (current). OPT copy, passport copy,2 photoes, check. Anything else?
