RE: How to give notice on leaving an employment?

I am leaving an employment that is not working out.  I started this job two weeks ago.  Many people who were hired for this or similar position leave after one month or two months depending on the boss assigned.  So human resource sees this happening a lot. 

1. What is the best thing to say to human resource and my direct boss?  

Since human resource sees this happening a lot, I probably should not give any pretense?  I mean, I probably should not say I am leaving for family reasons?  Could anyone tell me what is the best thing to say in my situation?  I want to make it really smooth and brief, esp. with my boss.  I want to make it brief since my email has not been set up yet, so I have to do this in person.  

Could I just say: I am leaving this employment.  My last day is xx.


2.When I give two-week notice on Monday, what is my last day?  Monday is the 13^th, but 27^th is the holiday.  So does my last day fall on the 24^th or 28^th?
