Just my two cents below.
1. It might not be a good idea to disclose your boss's cheating case. Wouldn't that put you in the same dishonesty situration as well, since you've known it from the beginning? And it would make it look like you're just taking revenge on your boss, since you decided to confess it right before you leave.
2. If you want to say something to your CEO about your boss, say the things that are related to the work. As far as your boss's health condition, it doesn't have anything to do with the company, so probably just leave that alone.
