At week 12, she will have her firest ultra-sound scan, and Nuchal test. Nuchel test involves the measurement of the water behind fetus' neck, if it's thicker than 2.5mm, there could be something wrong with the fetus; Your wife's blood will be tested as well. Her OB will combine the thickeness of the water, your wife's blood test result, your wife's weight and her age, to work out the chance of the fetus is Down. If the chance is less than 1 in 150, no further test will be needed; otherwise, your wife could have a CVS done straight away, or at week 16 to have Amnio done.
If the thickness of the water behind the fetus's neck is thicker than 2.5mm, you have to be prepared for that the baby has other problems. Your wife should have more scans done, at week 16, 20 and 24, for detecting other congenital problems.
Folic Acid is very important, never miss one.
